Your consent form provides written information regarding the risks, benefits, and alternatives of the procedure you are about to undertake. This material supplements the discussion you have with your healthcare provider. It is important that you fully understand this information, so please read this document thoroughly. If you have any questions regarding the procedure, ask your provider before signing the consent form.
I understand that permanent makeup is a form of tattooing that requires the implantation of pigment into my skin using a needle.
I acknowledge the potential risks and hazards related to this procedure, including but not limited to: allergic reactions to pigment or other products, lightheadedness , bleeding, bruising, swelling, scarring, and infection.
Publicity Materials
I authorize the taking of photographs and videos for educational, marketing, and insurance purposes. I hold the practitioner harmless for any liability resulting from this production and waive my rights to royalties or fees related to these materials.
Right to Discontinue Treatment
I understand that I have the right to discontinue treatment at any time, but full payment is still required.
All services, booking fees, and gift vouchers are non-refundable and non-transferable.
Gift vouchers cannot be exchanged for cash.
By booking with The Beauty Dash York, formerly KD Brow Bureau, you agree to these terms and conditions.
Payment
I understand that this is an elective procedure, and payment is my responsibility, expected at the time of treatment either by cash or bank trassfer only.
I acknowledge that permanent makeup removal may require surgical procedures, which may result in permanent scarring.
I certify that I have been fully informed of the risks of tattooing, including infection, scarring, potential difficulties in detecting melanoma, and allergic reactions.
I confirm that the shape drawn before the procedure is what will be tattooed onto my skin and has been fully agreed upon.
I understand that permanent makeup is a two-step process and that a perfecting session is necessary for optimal results. The perfecting session should be scheduled within 6-8 weeks. If scheduled after 8 weeks, it will be charged as a color boost of £99. Appointments held after 12 weeks will be charged as an annual color boost.
Booking Fees & Policies
Booking Fee:
I understand that no booking is confirmed until the booking fee has been received by The Beauty Dash York, formerly KD Brow Bureau. This fee is in exchange for a reserved appointment slot and is non-refundable. If I am unable to attend, I forfeit my booking fee. Any attempts to withdraw this payment may result in legal action.
Cancellation Policy:
A 50% booking fee is required to secure your appointment. This fee is non-refundable under any circumstances. However, if at least 72 hours' notice is given, you may reschedule your appointment once without losing your booking fee.
If less than 72 hours' notice is given, the full treatment value will be charged. This policy ensures fairness to all clients and compensates for preparation time allocated exclusively for you.
By booking, you agree to these terms.
Late Policy:
If you are more than 10 minutes late, your appointment will be canceled, and your booking fee will be lost. A new booking fee will be required to reschedule.
Appointments are scheduled back-to-back. If you are late, it affects other clients and may compromise your results. Please allow extra time for travel, parking, and finding the studio.
